I have been using Survey Monkey for almost two years and I am very happy. The survey builder is simple and easy to use, the data collection process is excellent and the report extraction tools great. 4 Thumbs UP!
Ok if you're willing to pay
Survey Monkey does surveys well, but it has it's cons. Their free service is very limited, but if you're willing to pay you're going to get a very good product. Basically, to me, the cons are the limitations of the free version.
Free Version:
If you're looking for a free survey program without the limitations, I'd suggest Google Docs Forms. The forms/questionnaires are embeddable and emailable(?), and you aren't limited by number of questions and responses. But if you're willing to pay, go with Survey Monkey.
